• Comprehensive final reports ranging from 8 to 12 pages.
  
  
• Special Report: Electronic Range of Motion Testing with improvement scales through data and graphs. AMA 5 th Edition Guidelines, adopted by the state of California notes that Range of Motion testing must be performed utilizing a duel inclinometer. Otherwise, average/eye ball ROM testing is invalid. Our office utilizes the Myo-Logic Functional Outcome Assessment System MSM 7000 Computerized Duel Inclinometer.
  
  
• Special Report: Electronic Muscle Testing with Reports and Graphs, Our office utilizes the Myo-Logic Functional Outcome Assessment System MSM 7000 for testing isometric muscle strength.
  
  
• Special Report: Radiological Reports. X-rays are read by a licensed Chiropractic Radiologist, D.A.C.B.R. X-rays are scanned directly on the comprehensive reports.
  
  
• Comprehensive S.O.A.P. charts are taken on a daily basis. All charts including initial intake forms and S.O.A.P. charts will be sent out with the reports.
  
  
• Impairment Ratings. Due to the adoption of the AMA 5 th Edition Guides to Evaluation of Permanent Impairment in the state of California in 2005, impairment ratings must be performed via the 5 th Edition Guides.

Due to our efficient office procedures, all reports and charts mentioned above will be sent with 2-3 days after the patient is discharged.
